      Book Description

Every morning, more than 50 million Americans wake up to another day in pain. If you are one of them, you know the way chronic pain can take over your life: the lost work and missed pleasures, the harsh drugs or surgeries, the feelings of anger and frustration, the endless search from doctor to doctor. It doesn’t have to be that way.

This remarkable book, by one of the leading pain specialists in America, puts in your hands the resources usually available only at the most sophisticated pain clinics. Recent research has shown that the solution to the puzzle of pain is almost never one “magic bullet” treatment. What works is a combination of therapies--often three or more--customized to your individual needs. Some drugs or surgeries can actually make your condition worse, while surprisingly gentle alternative therapies can provide measurable, lasting relief. The key is knowing how to combine the best of both conventional and alternative medicine.

Dr. James Dillard offers you what few physicians today are trained to provide:

      • fascinating new information on how pain affects your body and mind--including six myths that can keep you from healing
      • a detailed guide to more than 40 treatment methods, from state-of-the-art microsurgery and pharmaceuticals to acupuncture,
      yoga, and biofeedback
      • individual chapters on the 12 most common types of pain, from arthritis and back pain to fibromyalgia and migraines--clearly indicating what works and what to avoid
      • The Pain-Control Diet--and 20 top supplements for pain relief
      • how to build your pain-management team and get what you need from your insurance company
      • checklists, self-evaluations, and guides for tracking treatment
      • special chapters on children, seniors, pregnant women, terminal pain--plus how you can help a loved one in pain

You can take back your life from pain. The Chronic Pain Solution will guide you every step of the way.

      5 out of 5 stars Full Of Pain-Free Alternatives.......2004-03-16

This is an exceptional book for Chronic Pain sufferers seeking modern day therapy and answers to all of their questions which most doctors have no time to answer. Dr Dillard is incredibly knowledgable and covers all avenues to help you and your doctors agree on what treatments are right for you. If you suffer with Fibromyalgia, Chronic Fatigue Syndrome, Migraines, Irritable Bowel Syndrome, Arthritis or other debilitating pains, then I highly recommend this very positive and informative book. You will refer to it over and over again.

      5 out of 5 stars Dr. Michael L. Johnson.......2004-02-29

An excellent book!!! Dr. Dillard does a great job with his in-depth explanations of chronic pain and its various treatment protocols. In my 21 years of private practice have treated many cases in which patients who suffered from chronic pain were accused of "faking it" by their doctors. Chronic pain is REAL!! In my humble opinion, medicine does a great job in the treatment of acute care ( broken bones, heart attacks, strokes, etc.) but drops the ball when it comes to the chronic pain patient. Dr. Dillard shows you how to get back in the game!

Dr. Michael L. Johnson, author of "What Do You Do When the Medications Don't Work--A Non-Drug Treatment of Dizziness, Migriane Headaches, Fibromyalgia, and Other Chronic Conditions".

5 out of 5 stars Chronic Pain Management by James Dillard.......2003-09-16

This work is valuable in helping people identify modalities
for chronic pain. Oftentimes, the patient may need to assemble
a team of professionals in order to maximize the effort.
The author criticizes the reductionist/conventional approach
as being too harsh, invasive and ineffective. He stresses
the importance of evaluation and followup mechanisms to
manage pain on an ongoing basis. The author cites the
National Center for Complementary and Alternative Medicine as
an authoritative resource to help patients sift through the
maze of alternative medicinal options available to them.
For instance, Rolfing is a classical "deep tissue therapy"
for managing all kinds of neuromuscular problems and complaints.
Fascia manages connective tissue disorders through advanced
forms of stretching targeted muscle groups. The TENS mechanisms
are intended to block pain. This work is an important resource
to help patients identify the sources and strategies to
manage pain effectively on an immediate and ongoing basis.
The book is an important addition to any alternative medicine
personal library. It would be a perfect gift for any adult
interested in the alternative medicinal protocols.

5 out of 5 stars Holistic approach firmly grounded in modern medicine.......2003-07-18

Chronic pain has become perhaps the most trying and least understood health problem in this country. Millions of people suffer from chronic pain, and most of them have little hope that their pain will end. Many doctors are ignorant about the pain experience of their patients, and some are even dubious that the pain exists!

Part of the problem is that pain is an entirely subjective experience. It has no existence in the world outside of the pain sufferer (and the observable consequences of that pain). There are no instruments that can objectively measure pain. The plain fact, known only too well to chronic pain sufferers, is that nobody feels your pain but you, period. Dr. Dillard, however, among a growing number of progressive and caring physicians, realizes that the pain is real and has trained himself to appreciate and understand pain. On page 59 he even has a list of 68 adjectives for describing pain ranging from "aching" to "gnawing" to "wrenching."

Another part of the problem, as Dr. Dillard points out, is that many physicians have not had adequate training in pain management and tend to respond to their patients' needs from a narrow and relative uninformed perspective. A third part of the problem is that pain management is not a medical specialty. Most physicians simply are not experts in treating chronic pain.

I am not a chronic pain sufferer, but I know about this tragedy because someone I love has suffered for several years from near-constant pain of unclear origin. She has seen a number of doctors but has found little relief. To some extent she has become discouraged and cynical about ever finding help. I hope she will read this book and be inspired to renew her efforts to manage and reduce her pain.

Clearly this book does not offer a magic pill or any sort of miraculous solution. What it offers is an up-to-date and relatively thorough examination of ways to reduce and cope with chronic pain using methods combining alternative and conventional medicine.

Dr. Dillard's training and experience as both a chiropractor and a medical physician, and his open-minded and far ranging approach to the healing arts make this well-organized, well presented and eminently readable book the best I have read on the subject. Dillard begins with an attempt to understand pain using a whole body/whole mind approach. He encourages pain suffers to become part of a "pain management team" that includes the patient along with doctors, physical therapists and other professionals. His is a pro-active agenda in which he advocates "Taking Control of Your Treatment" (title of Part II).

Part III presents an evaluation of therapies from exercise, yoga, and meditation to acupuncture, pharmaceuticals, implants, and surgery. Part IV outlines the various complaints from headache through back pain and fibromyalgia to myofascial and complex regional pain syndrome. Part V focuses on "Special Considerations," pain management for children, pregnant women, the elderly and terminally ill. Dillard apparently has no prejudices against any given form of treatment, evaluating each according to its merits and appropriateness. He sees a place for methadone as well as meditation. Indeed, he usually recommends a combination of approaches.

Even though there is no magic bullet yet, and even though such a miracle may be many years in the future, Dr. Dillard's book offers hope that chronic pain can be reduced and managed (and in some cases eliminated entirely). People who are completely disabled by their pain can gain enough control to return to a more active lifestyle. Others who live only a reduced existence because of their pain may again find the joy in living. What Dr. Dillard's experience shows is that those who take an active part in managing their pain receive a synergistic effect by their active involvement. He emphasizes that immobility and lack of exercise (caused by the pain) tend to increase the pain experience, while exercise and an active engagement with life tend to reduce it. Consequently his approach for those whose pain is so bad they cannot exercise at all is to recommend even very strong pain medications so that exercise is possible, and then as activity increases to gradually lower the dosage.

This is an excellent first course in understanding and managing chronic pain and the sort of book that pain sufferers will want to read.

5 out of 5 stars Practical Advice For All Lifestyles.......2003-06-19

...This book is a gem. There is general pain-relief advice as well as chapters dealing in-depth with certain common conditions. I love that Dr. Dillard presents a range of treatment options, from the free to the very expensive, so money won't keep people from finding some relief. The book contains numerous references to help you find pain-relief specialists. In addition to having up-to-date information, this book is well-written and very positive.

Pamela Sheldon Johns is a connoisseur of the best Italian foods. Following up on her definitive books, Parmigiano!, about the queen of cheeses, and Balsamico!, about the artisanal vinegar that has enchanted cooks everywhere, Johns has written Pizza Napoletana! to tempt us with what is arguably the most authentic and best pizza in the world.

Neapolitans claim pizza was created in Naples during the 18th century. While it had plenty of forerunners (since every civilization growing wheat had some kind of hearth-baked flat bread), it is indeed a recorded fact that Antica Pizzeria Port'Alba, the first pizzeria, opened in the heart of Naples in 1830. Neapolitans are so fiercely protective of the quality of their pizza that, as Johns explains, a university professor assembled a 42-page document precisely detailing every requirement for making this specialty. He then spearheaded the movement which achieved a D.O.C., an official, government definition of what this pizza must be. Happily, la vera pizza Napolitana can made anywhere in the world, provided one meets these specifications for the flour, cheese, tomatoes, and techniques to be used.

Following a detailed history, and the explanation of the D.O.C. requirements, Johns describes how to make both the classic Marinara pizza, topped with tomatoes, oil, oregano, and garlic, and the true Margherita, a pie garnished with tomatoes, oil, mozzarella, and basil. In all, she provides 50 pizza recipes. For authenticity, some require the mozzarella di bufala used in Naples and also exported, while others use fior di latte, what Italians call cow's milk mozzarella. Still others are pizza bianca, like the Pizza con Aglio Arrostito, topped with just-roasted garlic and fresh rosemary, and pies made in other regions of Italy, such as Schiacciata, the Tuscan flat bread often called focaccia.

The work of making an authentic Neapolitan pizza is simple. However, for best results, either a wood-burning oven or a pizza stone to place in a conventional oven is called for. Johns explains how to deal with this. The many tempting color photos in Pizza Napoletana! can persuade you that her suggestions are worth pursuing. --Dana Jacobi

5 out of 5 stars The true story, beautifully presented.......2003-07-23

Having returned from 3 months in Napoli where I trained in pizza-making through Verace Pizza Napoletan, I discovered this book which accurately describes the way pizza is consistently made throughout Napoli. It's well written and designed, and includes great images, recipies, and insights into a wonderful tradition.

2 out of 5 stars Sabotaged by the Designer.......2003-07-02

As a long-time self-taught pizza chef--I've been struggling for 20 years or so--I'm always immediately attracted to any new book on pizzas. I keep hoping one of them will give me the secret of perfectly round disks of dough (mine usually resemble Australia) or, failing that, offer me fresh tips and little entertainment.

Pamela Sheldon Johns has worked hard at that, and to some extent she's succeeded. She's also been sabotaged by the pretensions of the person who designed her book.

Johns has done a good job of research; she's clearly gone to Naples instead of the Internet. And Naples is the font of real pizza. When I say "real" I am excluding those culinary crimes that come from franchise operations whose sole claim to fame is rapid delivery. I am also excluding that catastrophe known as "Chicago pizza," which in truth is nothing more than "bread with glop baked on it."

And so we learn about authentic ingredients--the right flour for the dough (and how to compensate for the fact that we can't get it here), the best tomatoes, the best cheese. We learn which kinds of pizza are now officially protected species in Italy (this is a complicated bit of legislation best left for Johns to explain). We get a good selection of nicely chosen recipes.

So what's to complain about? Well, the truth is this book is hard to read. Johns, like most cookbook writers (most specialists of any kind, for that matter) is not exactly a gifted stylist. OK, I can forgive the cheap trickery inspired by a too-long subscription to Writer Magazine ("Whipping in and out of the narrow alleys of Naples, weaving through traffic that blared a cacaphony of sounds, going down one-way streets the wrong way and against red lights, and amid the somewhat, and thankfully, incomprehensible words of the taxi driver slung out the window to his driving adversaries, I mustered the courage and vocabulary to ask . . . what is your favorite pizza?" I can even put up with "Of all the components [pizza crust] is the simple combination of flour, water, salt and yeast that makes it unique" (it comes so tantalyzingly close to making sense!). But so what? She tells us how to make really good pizza and doesn't waste much time in doing it.

The saboteur here is the designer, who believes books are to be looked at and admired rather than read and used. The photos are plentiful and attractive. The overall presentation is handsome. But it's hard to read. There's no excuse for using 8-point type (ordinary newspaper size) and for spacing the lines so far apart. The ink should be black, not wimpy gray. The ingredients lists are in eye-straining italics. And whenever possible, tiny italic captions are printed over dark backgrounds.

The reasons for this are 1) book designers like pretty, arty productions and 2) type interferes with their desperate, artsy pretensions. Useful type is legible: It tends to be large and black; it tends to be straight-up-and-down Roman, with serifs that contain the letters rather than let them bleed into the background. Type, in short, asserts itself because it is meant for the use and convenience of the reader. Type of the sort seen here--tiny, fussy, dim, vague--expresses the designer's self-regard--and his contempt for the reader.--Bill Marsano is an award-winning writer on wine and spirits, and travel.

      5 out of 5 stars Great Book!.......2002-05-09

This book has everything you need to make great pizza at home. It's beautifully presented, and the recipes are authentic, with enough variety to stop anyone from getting bored. In addition to the basic recipes, the mushroom and smoked mozzarella is delicious as is the shrimp and pesto.

2 out of 5 stars Beautiful Show - Meager Go.......2002-02-24

While this book is beautifully designed and produced, and has many stunning photographs, as a highly experienced pizza maker, we feel it is best left on the coffee table, and kept out of the kitchen.

Unfortunately, form and graphic design are the prime focus.

Master pizzaiolos will not lose sleep over this one; no secrets are revealed.

Those who wish to learn to make great dough are far better off reading Nancy Silverton.

      5 out of 5 stars A pizza lover.......2001-10-27

Try the receipes in this book. I used the flour mixture, fresh yeast, the slow rise of the dough. Like the author states, the Pizza is the dough. The results is very satisfying. We are used to piling all kinds of toppings on our American pizza. Using fewer toppings, one can taste each flavor. If you want to get serious about making the Italian Pie, then this is the book to buy. You won't be disapointed! The author has done her homework.

          5 out of 5 stars Stunning,,,,simply stunning.......2007-05-08

If music could be created with needles and yarn~ (and be able to be seen with the human eye )~then this ,most certainly ,would be what it would look like.

These garments are true works of art.

This book was published in the late 90's ~ but~ what is of particular interest, is that this book is laden with knits that are so jaw droppingly ( I think I unvented a word here) beautiful for TODAY,,,, that I had to sit and absorb them for awhile.

I would leave this lovely hardcover book out my coffee table,, and this is the first knitting book I can honestly say that about with complete truth.

These are what I call" recipe patterns" in the way that the knitter can use the recipe given for their own creations.

In essence, the fiber artist will create the fabric , fit and the design they love.

While there are some specific directions for sizes, the author is very clear at pointing out that any garment can be knit with an infinite number of variables~

She adds a variety of suggestions and truly fosters a feeling of knitting independence. The knitter then feels the freedom to create their own masterpiece,, and savor the process of designing.
The decisions made along the way are part of the process.

There is simply NO way that a knitter can compare works of Meg or her mother ( EZ) to the Step- By- Step Pattern Books of today.

They , along with Barbara Walker and a few select others ,,are in a class of their own.
These people TAUGHT knitting , and are teaching you to think along the way.

I often wonder if this Brand New Generation of knitters will produce one teacher of their caliber.
          I hope so.

Even IF you do not knit a single one of the garments in this book, consider purchasing this book for learning tips.

For instance,, I was not sure how to knit backwards,,,,now, I understand the process,,,,,and now I also am learning how to better make a hem on sweater work for me~ vs a simple ribbed bottom~
The list could go on and on.

I can buy basic sweaters from Lands End , but THESE are the sweaters I wish to spend my time creating!

One added note,,,,This book as well as ALL of EZ's books are published in the USA ~ (no offshore printing done)

          I absolutely love this book.

5 out of 5 stars Love these patterns!.......2006-04-09

I love knitted garments that draw from the designs of the Turkish, Russian, and Scandanavian cultures. The colors and textures used in this book are timeless. The construction techniques illustrated are indispensible. Needless to say, I am an avid fan of circular knitting and look specifically for books which cover this particular type of knitting whenever and wherever I can. As with any art, it takes time and effort to master technique. And really no one book is the best book for everyone to learn from. That's why it's wonderful that there's a demand for knitters to share their ideas and publish their designs. I applaud Mrs. Swansen for yet another winner. I use her books as a benchmark. If I can master the techniques in her books, I know I'm on the right track.

P.S. A word to the wise...please remember to knit a swatch in the ROUND first before attempting any circular knitting project. Tension is different in the round vs. knitting back and forth on straight needles.

4 out of 5 stars Technique great, patterns repetitive.......2005-09-08

I have to say that I was a little bit disappointed of the patterns. Nothing new, nothing exciting. Sure, the pattern seem to be "authentic", but I can find them in other books as well.
But the "how to" area is very well written and helpful even for an intermediate or advanced knitter.

          2 out of 5 stars Unispiring sweaters.......2004-02-09

I've flipped through this book countless times while browsing my local library's knitting section and I'm always turned off by the boring and frequently ugly designs and color schemes. This author apparently has never met an earth tone she didn't like. How about adding a little sparkle to some of those muddier colors with some jewel tones? I can't speak for technique having never used the book. My advise is to browse through it at the library or book store before purchasing it here. Put it this way--it never inspires me enough to check it out of the library---for free! Of course, I'm not a big fan of her mother's book, Knitting Without Tears (I can imagine the gasps of outrage since she seems to be a knitting godess) and wish I'd spent my money on something less boring. I've had Knitting Without Tears for nearly four years now and have yet to use it for anything. It's just wasting space on my shelf. Since this book is a much bigger investment be wise and borrow a copy before buying.

5 out of 5 stars Great book for all knitting knuts!.......2002-04-29

Loved the book - use it with her videos and without -- find it easy to understand compared to most other patterns available today. This book will never leave my library. Her teaching method allows me to design my own patterns without a lot of fuss. Writes in a way that makes me feel like she is sitting in the living room helping out. She simplifies everything she can and makes knitting friendly again. --And that doesnt mean she is making simple sweaters - it just means she is making beautiful sweaters easy to knit. Full price would be a bargain for this book! Happy Knitting!

4 out of 5 stars Informative and easy to read.......2007-05-31

This book has helped me to understand the different types of thyroid diseases better. I'm very glad that it also lay out the risks of certain surgery that some doctors do not highlight. It is written in a manner that is easy to read and not heavy as compare to scientific books.

              4 out of 5 stars Very Informative.......2006-08-21

This is a really go book for anyone who doesn't know anything about thyroid conditions/diseases. I have had some experience with thyroid problems in the past so a lot of the information contained in the book was not new to me. The best part about the book is that you get detailed information without all the medical jargon. The author is very clear and descriptive about the different types of thyroid conditions, symptoms, common medical treatments, etc. I purchased it because I was hoping for more information on weight management and diet for tyroid patients. Although the book touches on this topic it was not as detailed as I would have liked. Otherwise, I would recommend this book as a guide to walk you through the maze of medical terms and to sort out some of the fact from fiction.

3 out of 5 stars Nothing new here.......2006-05-27

Why did McGraw-Hill publish this book? It is almost identical (verbatim) to The Complete Thyroid Book except that the latter book includes some additional information. Both books were published in 2005.

I agree with the reviewers that mention how basic this book is. If you're new to learning about thyroid conditions and their treatment, The Complete Thyroid Book is a good starting point. Otherwise check out this book or The Complete Thyroid Book from the library and maybe you'll discover a few helpful tidbits.

              5 out of 5 stars Extremely Informative.......2006-03-22

I bought this book after a tough year of thyroid ups and downs. It has empowered me to understand and ask the right questions for a change. So informative but on a level that anyone can understand. After reading my copy, I bought one for my sister, who also has thyroid problems. Her mother-in-law saw it and bought two more copies for friends. Its great for family members of hyper-hypo- thyroid too. It will increase thier understaning. I bought a few books and this was the best, by far!

5 out of 5 stars Thyroid Sourcebook by Rosenthal.......2004-01-22

This is an excellent book which describes classic thyroid
problems and a variety of solutions. Goitrogens are contained
in cabbage, cauliflower, turnips, almonds and fiber.
People living in North America get natural iodine
in their diets. The thyroid makes T3 and T4 cells. The
              parathyroid controls calcium regulation. Hyperthyroidism
produces anxiety, diarrhea, easy bruising, increased gland
size, eye problems, exhaustion, heat intolerance and
palpitations, infertility, menstrual changes, muscle weakness,
paralysis and tremors. Hypothyroidism causes cardio changes,
cold intolerance, digestive changes, fatigue, muscle cramps, numbness and autoimmune deficiencies. The Endocrine Society
is listed at 4350 East West Hwy. -Suite 500 Bethesda MD 20814-4410 at Tel 301-941-0200

Overall, this work is excellent for anyone desiring quality
information on the functioning of the thyroid gland. It should
be in every personal health library.

When I was diagnosed with thyroid cancer [hurthle cell carcinoma] I was looking for a good book to learn more about this condition. This is that book. It is well written, in plain language, and I could understand everything written there. Thanks to Ms. Rosenthal for writing such a fine book. I have referred many of my friends and colleagues who have come to me for advice to this great resource.

After reading this book and comparing it to Dr. Arem's book The Thyroid Solution and another book for Graves Disease called Graves Disease; A Practical Handbook; I found this book to be lacking in how it was presented, ease in understanding and resourcefullness.

5 out of 5 stars Great for newly diagnosed **or** long-term thyroid patients.......2000-03-30

This book -- and the newly printed "Living Well with Hypothyroidism" -- have been pivotal in my experiences as a hypothyroid patient. Perhaps the most important thing I've learned is that newer research shows that many thyroid patients feel better with a medicine that contains two types of thyroid hormones (T4 and T3). Currently many of us are still on pills like levothyroxine or Synthroid, which only have one type of hormone. Although I haven't yet convinced my HMO doctor to change my medicine from the traditional Synthroid, she did increase my dose and I already feel better. This gives me hope that I can feel even *better* on a medicine with both T4 and T3.

I am a layman that has studied the thyroid for many years. Some of the information that she presents is false, a novice would be easily misled on some very inportant guidance. On p. 3 she correctly states that iodine deficiency can cause a lot of the thyroid problems, but on pg. 85 she states that Asian women who move to Ca. are at a higher risk because they are now in a iodine rich diet. Completely false. They moved to a area that is extremely low in iodine. In the U.S. the average intake is under 100 mcg. per day, verses 13 mg. in Japan. Japan has the lowest thyroid and breast cancer in the world.

                      Book Description

As the visionary behind the planned community in Radburn, New Jersey, Clarence Stein was heralded as one of the most progressive and controversial American architects and planners of the twentieth century. Stein's admirers placed him in the company of such giants as Lewis Mumford and Benton MacKaye. He championed green-centered, pedestrian-friendly, dispersed residential communities, finding inspiration in his studies in Paris as well as the Garden City movement of Great Britain. His ideas influenced well-known developments in Greenbelt and Columbia, Maryland; Reston, Virginia; and Woodlands, Texas. His collaboration with Benton MacKaye in the Regional Planning Association of America led to building the Appalachian Trail, America's prototypical greenway. His work has influenced community planning all over the world -- including Tapiola, Finland, and other post-war towns of England, Scotland, and Sweden. Stein's dramatic personal and professional story, however, has remained largely unexamined until now.

In The Writings of Clarence S. Stein: Architect of the Planned Community, Kermit Carlyle Parsons presents a wide-ranging selection of more than 500 annotated letters, papers, and other writings that shed light upon the personal struggles and professional achievements of this major force for change in community planning and regional design. Parsons supplements these documents with a succinct biographical introduction to Stein's life and career, 137 illustrations (including photographs, plans of Stein's work, and personal sketches), a complete list of his many projects, a bibliography of Stein's own articles and books as well as articles about him, and biographical sketches of the people mentioned in the documents. The Writings of Clarence S. Stein offers the first full examination of this American whose city planning ideas transformed communities in both the United States and Europe.
