          I was asked to choose a book from a list of books on problem formulation to report on for one of my classes in my MBA program. I ended up choosing this book. I was at first excited to see a different point of view on problem solving than the one we were discussing in class. However, I was very disappointed with this book.

          The book comes out right up front and states that this is not meant to be an academic book, but a tool that can be used in helping define the root cause of problems. This book although great for the amount of tools to aid in finding root causes, seems to lack content in total problem formulation. The layout of the book flows nicely so that if you had a particular problem that you were working on you could easily go through step by step to work on finding the root cause. However, it doesn't focus enough on critical steps in problem formulation, such as the beginning steps, determining if there is actually a problem, and properly framing the problem (scope and domain). It also doesn't focus enough on the final steps to solve the problem such as generating a hypothesis and solution, and controlling the results so to make sure the problem does not re-occur.

          In my opinion there are much better books out there to discuss problem solving.

          I found this book most helpful in trying to learn quickly the techniques of solving cause and effect problems; to which I had no previous study or training other than common sense.

          It gave me specfic ways of gathering information and using it as a basis of making decisions that effect our daily operation and to be able to find ways of improving the quality of what we do.

          After looking through many books on root cause analysis, I came across this books published by the American Society for Quality (ASQ). While most books on root cause analysis go deeply into the theory and the concepts of the topic, this book presents, as the subtitle states, simplified tools and techniques that can be easily applied to any organization. At my employer -- a medical device company --, we have trained over 100 employees using the Andersen/Fagerhaug book to solve technical as well as staffing issues. I highly recommend the book for use at all levels of an organization.

          This book is useful for anyone starting out creating a root cause analysis process at work. It explains the difference between a root cause and what most people think of as a cause. Unfortunately it does not delve further into a methodology for addressing more complex problems and a standard for true root causes in more complex events. Any RCA program needs to identify patterns over time, not just individual incidents. Categorization and clear justifications are needed, particularly in technology. For example: A computer component breaks and the network does not fail over to a backup. Is the root cause the component or the failure of the architecture to operate as designed? If either one had worked the customer would not have experienced a problem. Both issues, of course, have to be addressed. But when you examine patterns over time do you see that your hardware vendor needs more attention or your network architecture?

          Also, it would be helpful to differentiate trigger events from root causes. If a pilot has a heart attack on a commercial airliner but the co-pilot is unable to take over and crashes the plane, is the root cause the heart attack or the lack of skill of the co-pilot? He's there to assist the captain and to take over if something happens. He should be able to fly alone if necessary. Yet he could not safely fly the plane. The pilot's death would appear to be only a trigger event and not a root cause at all.

          Another example: A network cable was not seated correctly when the network was built. Thousands of cables were connected correctly, one was not. Random human error? Should you delve into the person who did the work? Should one human error be investigated down to that level given how many cables were done correctly? Would the answer change if the lose cable caused the firm to lose millions in online purchase orders? How much time and effort becomes diminishing returns over time? Where does an enterprise get the biggest payoff?

          None of this diminishes the value of this book if your firm is becoming more aware that individual service outages and product problems need to be addressed to improve the customer's experience at a reasonable cost of those services. This book will show you how to delve further than the obvious to identify what really affects your business.

          This book gives you all the tools you need. TRY IT, you'll like it. I have seen it in practice and its awesome. It walks the novice through what it took the rest of us years to do, mistake proofs the process.

          If I would have had the opportunity to view this book in a bookstore I would not have purchased it. The first three chapters all discuss why you would want to perform RCA and who should do it. Chapter 4 provides a very basic overview of tools you can use for RCA. Chapter 5 is all about a basic checklist and chapter 6 is fluff with two generic examples.

          Unless you know nothing about quality management systems my advice is to save your money and spend a few minutes searching the web. You will find better information for free.

          Denise Robitaille's book is of great value to anyone in search of a guide for determining true root cause. The book offers the reader an abundance of logic and common sense, presented in a conversational, easy to understand manner. Several of the classic tools, (the 5 why's, cause and effect charts, etc.) are well represented and linked to a new, user-friendly "Root Cause Analysis Checklist." This approach guides the root cause inquiry process through twelve primary areas taking into account every major section of the customer supply chain.

          Determining true root cause within every company remains a classic weak point in the corrective action process. Any organization in search of a well written, user friendly and practical approach to the subject should read this book.

          I bought this book as a review book over Christmas. While it is useful in outlining the advantages and disadvantages of each method there are few examples. This book will be useful for your second shelf but did not earn a place in the top shelf. "Quality Improvement Tools and Techniques" by Mears fills that spot. However, Mears' book does not cover RCA in any detail at all so this book still earns a place in the second shelf. A much better book for RCA would be Smith's "Reliability, Maintainability and Risk: Practical Methods for Engineers." He provides more examples, which are the best way for engineers to learn. I do not keep it in the top shelf because it is too detailed. If you want an overview, Wilson's book may be useful; when you get confused, go to Smith.

            My feeling is that the target audience for this book is practitioners who did not study strategic management back in school but want to adopt a strategic thinking in their businesses. The book would be a good read for that purpose. This book was one of my first readings in my graduate level research involving strategic management. While I enjoyed reading it, after reading other strategy books like Aaker's "Developing Business Strategies", Porter's "Competitive Strategy" and Mintzberg's "The Strategy Process", I never had to go back to this book again. The process proposed by the author is essentially an optimization process to find the optimum solution, which they call "breakthrough strategy". However, the book falls short of demonstrating how the proposed process can be used to address real life problems which are often complex and dynamic. Even though the authors claim they were able to formulate consistently breakthrough strategies in the past using this process in real life, I think the book does not provide enough evidence to support that claim. On the other hand, I also acknowledge that there is a need for structured processes to help planners to formulate effective strategies even if they are not tested thoroughly. The book is a good attempt to meet that need.

            I am the technologies director of my firm, and I have read many books on the subject of strategy formulation and innovation management, as well as a host of other business subjects. I find that they all have one common flaw, they do not provide answers to the thought provoking questions they ask. Business Strategy Formulation, on the other hand, has all of the answers to some of today's biggest challenges in business. Mr. Ulwick obviously has a tremendous understanding and practical knowledge of creating breakthrough strategies and solutions for organizations. He demonstrates this with incredible case histories and his detailed explanation of problem areas. Additionally, Mr. Ulwick explains his ground-breaking new technology with precise terminology, leaving no room for question. When I was finished reading this book, I felt that I had, for the first time ever, a clear understanding of what strategy formulation should be. I also finally understand why it is so difficult to grasp what my customers value and how to deliver that value. This book is a must read for anyone interested in learning more about strategy formulation, managing innovation, or creating customer value.

Did events and eras such as the crucifixion of Jesus Christ, the Roman Empire , the Dark Ages, and the Renaissance, actually occur within a very different chronology from what we've been told? Yes, they certainly did!

            The history of humankind is both drastically shorter and dramatically different than generally presumed.

            Why is it so? On one hand, it was usual custom to justify the claims to title and land by age and ancestry, and on the other the court historians knew only too well how to please their masters. The so called universal classic world history is a pack of intricate lies for all events prior to the 16th century. World history as we learn it today was entirely fabricated in the 16th-18th centuries. It's likely that nobody told you before, but

            there is not a single piece of firm written evidence or artefact that is reliably and independently dated prior to the 11th century.

            Naturally, after what you've learned in school and university, you will not easily believe that the classical history of ancient Rome, Greece, Asia, Egypt, China, Japan, India, etc., is manifestly false.

            You will point accusing finger to the pyramids in Egypt, to the Coliseum in Rome and Great Wall of China etc., and claim, aren't they really ancient, thousands of years ancient? Well, there is no valid scientific proof that they are older than 1000 years!

            The oldest original written document that can be reliably dated belongs to the 11th century!

            New research asserts that Homo sapiens invented writing (including hieroglyphics) only 1000 years ago. Once invented, writing skills were immediately and irreversibly put to the use of ruling powers and science.

            The consensual chronology we live with was essentially crafted in the 16th century by the Jesuits.

            The world history was compiled from contradictory mix of innumerable copies of ancient Latin and Greek manuscripts and other irrefutable proofs delivered by late mediaeval astronomers that were cemented by the authority of writings of the Church Fathers.

            Early in life, we learn about ancient history. Children love the magical lessons of history - they are like fairy tales. Teachers recite breathtaking stories; very soon We learn by heart the names and deeds of brave warriors, wise philosophers, fabulous pharaohs, cunning high priests and greedy scribes.

            We learn of gigantic pyramids and sinister castles, kings and queens, dukes and barons, powerful heroes and beautiful ladies, emaciated saints and low-life traitors.

            Ancient history is based documents, manuscripts, printed books, paintings, monuments and artefacts - called primary sources.

            The problem is that neither these ancient documents, nor events described therein can be irrefutably dated, moreover they contradict each other for the most part.

            When a school textbook tells us that Genghis Khan in year X or Alexander in year Y, have each conquered half of the world, it means only that it is so said in some of the written sources.

            There are no answers to simple questions:

            When were these primary sources written?

            Where and by whom were these sources found?

            It is wrongly presumed that ancient and medieval chronicles, written by Genghis Khan's or Alexander the Great contemporaries and eyewitnesses, are readily available. Actually, only sources written hundreds or even thousands of years after the events are there, compiled mostly in the 16th 18th centuries, or even later.

            As a rule, these sources suffered considerable multiple manipulations, falsifications and distortions by editing. At the same time,

            innumerable originals of ancient documents under various pretexts were destroyed in Europe under various pretexts.

            The names of persons and geographical sites often changed meaning and location during the course of the centuries.

            Geographical locations became clearly defined on maps only with the advent of printing.

            This made possible the circulation of identical copies of the same map for purposes of the military, navigation, education and governance tasks.

            Historians from Oxford say: "hey, everybody knows that Julius Caesar lived in the first century B.C.

            `Julius Caesar' statement is only a point of view as

            there is simply no irrefutable documentary proof that Julius Caesar or any other great name of antiquity ever existed.

            Better than that - extremely rare sources that can be reliably dated back to the 10th-14th centuries A D, do not show the polished picture of classical history.

            They show a picture both contradictory and confusing.

            All methods of dating of ancient sources and artefacts are erroneous:

            Radio-carbon C14 method produces dating with exactitude of plus minus 1500 years, therefore it is too crude for dating of events in historical timeframe!

            The Almagest tractate, which lies as corner stone contemporary chronology, compiled in the 2nd century A D by Ptolemy, the founding father of astronomy, contains astronomical data of 9th to 16th century!

            The Bronze Age,that has supposedly began 5000 years ago. Bronze is made of 90% copper and 10% tin, but the technology for tin extraction dates back to 14th century A D!.

            All eclipses contained in manuscripts, like Thucydides one, relating 'ancient' events have exclusively medieval dating. All horoscopes cut in stone or painted in Egyptian temples, like Dendera have exclusively early medieval dating solutions.

            Not quite what you have learned in school? Open your eyes, and, you will find sufficient proof to reach step by step the inevitable conclusion that the classical chronology is false and therefore, that the history of ancient and medieval world universally accepted today, is also false. Have a fresh outlook on everything said or printed about "ancient" and "enigmatic" Roman, Greek and Egyptian, medieval as well as all other "lost and found" civilizations.

            Antiquity and Dark Ages are phantoms invented in the 16th 18th and polished in 19th 20thcenturies. Human civilization is in fact barely 1000 years old!

            Yes, we can all agree that mainstream history is nearly 100% BS due to politics, economics, ego, problems with dating techniques, and various conspiracies. Agreed. But, I've been researching the distinct possibility that human history (in terms of civilizations) are much more ancient than we've been told, so coming across this book was very interesting to me. I wondered how Fomenko could be wrong (if at all) because he is very persuasive in his presentations. Then it dawned on me. If at previous times in prehistory, due to the various catastrophies that are well documented (comets, asteroids, planetary disruptions, plasma discharge, pole reversals, etc) the Earth was in a different position in relation to the sun, different tilt on its axis, different orbit, different rotation (in terms of velocity and DIRECTION), and the continents were in different positions, then would this not cause the ancients to see the sky (constellations) differently? In other words, is Fomenko making erronious assumptions about the physics of the Earth in pre-history, which then corrupt his data with regards to dating the relevant astrology? The last event to seriously disrupt our planet occured roughly 3500 years ago, according to other good researchers, so is it possible Fomenko has been confused by this? The vastly different physics of our planet in the not so distant past may explain this confusion, which is not to say the "mainstream" version of history is correct; on the contrary. I am not an expert in these fields, but wanted to see if this idea could spark discussion.

            Will people ever read before spamming? Yes, Jesuits could not rewrite world history alone, they had help. Anyway, Dr Prof Acad A.Fomenko does not point to jesuits as the driving force of world wide history manipulation in published volumes 1,2,3;, actually he barely mentions the poor devils. Check it with 'Search inside' feature, please. China is rarely mentioned either, in fact, Dr Fomenko is completely eurocentric. Right, his theory contradicts all mainstream schools of history, because in their actual state they are all built on blatantly erroneus chronology. You don't need a mysterious cabal (conspiracy) to falsify history, the falsification is its modus operandi. It is inherent to history(ians) to falsify (distort) events, as it is inherent to humans to boast as it is inherent to power (authority) to legimize itself by referrring to glorious past made to its own order. Dr Prof Fomenko and team have identified scores of instances of such manipulation in Russian, European, etc.. history, and delivered valid statistical proof thereof. His own 'reconstruction' is completely another story. Forget c14 as a valid method of dating. W.Libby has initially discovered a brilliant method of INDEPENDENT dating. Too bad, c14 method has become a joke after a forced marrige with dendrochronology with consensual chronological scale inbuilt. Radiocarbon method can't stand blind tests, but is so very productive as a rubberstamp.

            There is no doubt that history as most know it is a sham, & institution's version of History both University & Church is fradulent & inaccurate. Everything was established with an agenda, The real "Dark Ages" are now when we have access to incredible amounts of information past authorities & more important 'common folk' didn't have but our institutions & educators are slow to evolve because of what has ignorantly & arrogantly been taught for too long. This is on many subjects not just Chronology.

            For anyone to question "Why would a Mathematician have anything credible to say of History?" The answer is from Dr. Fomenko's preface in the book: "It would be worthwhile to remind the reader that in the XVI-XVII century Chronology was considered to be a subdivision of Mathematics." These volumes could possibly be some of the most important works to date & should be read by everyone with an interest in History, especially professors & educators who have a duty to the public. I have read both books & must say that 'Chronology 1' has some very eye opening & revolutionary information. Even if these volumes are part true the implications are profound & opens the doors to further investigations & questions which must be done. I speak several different lanquages & must say the logic Dr. Fomenko uses with "inflection" of words & words being read from left to right in one region & right to left in another then written backwards, the removal of vowels & get down to basics of words, or different cities & locations having the same name etc. is correct. Vowel usage has always been optional & varied, actually complicating linquistics & study. The first thing one has to understand is that words never had a fixed spelling in history like we do now, the spelling of words was mutable & regional, as well as names & titles of people were vast, varied & changed, NOTHING WAS FIXED or understood linear. Matters of Life & Death as well as financial profiteering yesterday & today were & are made with ignorant, illogical & conspiratorial views of history & reality, it's time people get closer to the Truth & society collectively grow up.

              Howard Bodenhorn's State Banking in Early America studies the financial experimentation that took place in the United States between 1790 and 1860. Dr. Bodenhorn's book explores regional differences in banking structures, which bear indirectly in the conection between financial and economic development. If a single theme emerges, it is that the United States benefitted from its free banking philosophy in which state governments, rather than a centralized authority, created financial structures designed to serve specific, local needs. Thus decentralized federalism provided state legislatures with a great deal of flexibility in their individual approaches to economic and financial issues.                   5 out of 5 stars GREAT BOOK ABOUT ADVERTISING AND LIFE.......2004-04-19

                  Like the author, I have worked in the creative side of advertising for 20+ years and feel the same frustrations he did before he started his own company. That was he was able to venture out and chase down his dream of independence provides hope for us all, I suppose. There are many stories, impressions and advice-based advertising lessons but I think what I enjoyed most was reading about how the move allowed him to create a unique balance of family/work that lets him do more for both. I may not be able to leave my job today and do what he did. But this book certainly gets me thinking about what my next "big" move will be and whether I want to forever work for someone else. The tone of the text has an easy-going pace and relaxed humor, but it's not lightweight at all. Rather, the writing provides a good study and solid foundation for all of us "creative types" who are ready to spread our wings more not just in the office but at home with our spouses and children, too. I shared this book with others at the agency where I work and everyone took something good away from it, from our "rookies" to a few seasoned vets. I think this is an excellent book for anybody in advertising, and other businesses, too. After all, when we hit 40 and start to wonder about "what if", it really doesn't matter what we do for a living--it matters what we decide to do with our lives.

                  Toward the end of Chris Epting's book, he muses as to whether the reader will come away with any kernels of knowledge or wisdom regarding what he's input, in diary form, of a copywriter, author, photojournalist, historian, lecturer, star gazer, husband, father, mentor and friend who has struck out on his own, after 20 some years under the corporate thumb. By then it's too late to wonder. By then the reader is so filled to overflowing with enthusiasm about the possibilities in his or her own life that finishing the book and having at it is the prime concern and goal. This is a fast, thoroughly enjoyable read that mixes the balancing act of intertwining family and work life with a road not taken first hand account of a trapeze act without benefit of net. And in between the day by day account of Mr. Epting's own first year, we get juicy tidbits of corporate gossip, historical ad biz references, and a forty year old's Doogie Howser-like philosophical musings on all manner of life's bits and bobs. Going through your own mid-life crisis? Don't go nuts, focus it into some form of your own life's calling to something like this. Have you got what it takes? Have you considered all it takes? I doubt Mr. Epting even considered this book or his life as either template or blueprint, but what he's done in "Diary of an Adman" is give the reader an inspiring journey, a steeping stone, if you will, to find their own opportunity to do what Chris Epting seems most to enjoy... to dream.
